Adult Case Manager - FT, St George

Starting Wage: $16.91/hr (+DOE)

GENERAL PURPOSE:

Performs a variety of professional duties such as skill development, behavior management, targeted case management services as needed.

SUPERVISION RECEIVED:

Direct Supervision: Provided by the Program Manager or Staff Supervisor. Supervision includes task assignment, work performance, job expectations and regular activities.

SUPERVISION EXERCISED:

None

ESSENTIAL JOB FUNCTIONS:

  • Completes all necessary paperwork/documentation including assessments, safety plans, progress notes, correspondence, and other required documentation in an accurate, thorough, and timely manner according to Center mandates.
  • Completes all required training, including E-learning assignments within assigned timeframes.
  • Attends and actively participates in all assigned meetings, such as staff meetings, team meeting, committee meetings and community meetings.
  • Participates in the development of client goals and objectives. Works with clients and the clinical team to facilitate accomplishment of each client's goals and objectives.
  • Participates in client progress evaluations during team meetings, reflecting observed progress of clients and verbal feedback from clients
  • Works to build and maintains knowledge of community resources.
  • Skillfully provides Targeted Case Management (TCM) assuring that the TCM services provided meet the criteria of either Assessing TCM needs, Coordinating and Linking to services or Monitoring progress in meeting TCM objectives. (Must be certified as a case manager)
  • Efforts are made to assist the client in becoming more resilient.
  • Skillfully provides Certified Peer Support Specialist (CPSS) objectives, using a curriculum to guide the teaching of the skills and documenting progress towards the specific objectives.
  • Skillfully provides behavior management teaching proven techniques for addressing specific behavioral objectives, such as anger management techniques to improve anger control.
  • When assigned, works with clients and community businesses to help clients obtain resources to meet specific client needs.
  • Uses non-violent crisis intervention to diffuse client escalations.
  • Transports clients to and from activities or appointments, and completing all necessary forms before and after transporting.
  • Performs related duties as required.

PREFERRED QUALIFICATIONS

Certified or ability to become certified in Targeted Case Management (TCM) and/or as a Certified Peer Support Specialist (CPSS).

MINIMUM QUALIFICATIONS:

1. Education and Experience

A. High School Diploma or GED required. Graduation from a college or university with a bachelor's degree in social work, psychology or a related field preferred.

B. Three (3) of months of experience working with mentally ill adults or some other social service program preferred.

2. A valid motor vehicle driver's license (equivalent to State of Utah Class D)

3. Able to pass background check (BCI)

4. Preferred Knowledge, Skills, and Abilities

A. Some knowledge of:

  • mental health and substance use treatment program resources
  • individual and group counseling skills
  • crisis intervention methods
  • recognize and provide community and regional resources

B. Ability to:

  • develop effective professional working relationships with co-workers, supervisors, clients, and the public
  • communicate effectively, verbally and in writing
  • establish priorities
  • work independently and manage work assignments in a high interpersonal contact environment
  • maintain appropriate professional boundaries
  • follow ethical guidelines and Center/Program policies, procedures, and protocols

C. Demonstrates skills in:

  • establishing priorities
  • working independently, and proceeding with objectives without direct supervision when appropriate; creatively handling and resolving problems
  • time management practices encompassing all job duties, such as scheduled appointments, meeting obligations and paperwork
  • conflict resolution; able to handle and resolve recurring problems

D. Must be proficient in use of computer and have keyboarding skills in order to enter and retrieve client's electronic records. General knowledge of Microsoft office software, such as Outlook, Word, Excel, and PowerPoint are preferred.

4. Special Qualifications

A. CPR/First Aid certification (within 6 months of hire and upon expiration)

B. Certification in Non-Violent Crisis Intervention (within 6 months and annually thereafter)

C. Working schedules must be flexible to meet consumer needs. Staff must be willing to work a variety of schedules to accommodate various client needs.

D. Physically able to lift up to 40 lbs., climb stairs, transport clients, etc.
